IMPORTANTE

Ai fini dell’ottenimento dei crediti ECM è assolutamente indispensabile compilare in ogni sua parte (in particolare nome e cognome, data e luogo di nascita, codice fiscale e indirizzo e-mail) il Modulo Dati Personali, la Scheda di Valutazione del Gradimento ed il Questionario di Valutazione dell’Apprendimento.
